LOW INCOME SPAY-NEUTER PROGRAM APPLICATION


INSTRUCTIONS FOR SENDING IN FORM (do not send these instructions with form)

1. Fill out form and sign it.

2. Mail to:
Spay and Neuter Program (Application)
PO Box 550
Gallup, NM, 87305

3. Wait a few days and then call for an appointment. They do it on Wednesdays twice a
month.

4. Take your animals into the shelter at 9 am on the day of your appointment, and pick them
up the same day. No food or water after midnight before the surgery.

5. Be sure to tell them before the surgery if it is a female feral (too wild to handle) cat or
Dog and they will use stitches that do not have to be removed (they will dissolve over
time). Please ask for instructions about how to remove the regular stitches within 7 - 10
days after spaying (only females have to have stitches removed).

6. If you cannot afford to pay the amount requested ($35), pay what you can at time of
payment. Please pay something, if you can, so others can have their pets spayed or
neutered.
